Accident Reports
When an accident occurs, for example an injury to a player, it is important that you fill in an accident report. We require this so that we are aware of any accidents, and can record them for future reference if required.
You can download an accident report below, which you can print and fill in when required. Completed accident reports should be returned to the Student Activities Coordinator.
